Request for Information – Faculty self-attestation form language

Today, January 16, 2023, the SCFA submitted to Deven Desai, ELR Director, a request for information for the complete language of each stage of the faculty self-attestation form.  The google form that was created by Labor Relations for faculty does not allow faculty to see the entire form. Please note that this request did not indicate SCFA’s consent to these forms.
